Which visit type you want (or a brief description of what you are looking for if you are not sure)
Your general availability
Whether you want in-person in Corpus Christi or virtual
A note on privacy
Your date of birth is considered protected health information. Text and email are convenient but not encrypted end-to-end. If you prefer to keep this information over a secure line, call me instead and I collect the details on the phone.
